Services that are reasonable and necessary for the diagnosis or treatment of illness or injury known as "medically necessary."

Medical necessity is based upon your ability to objectively document "functional improvement." I hear from chiropractors almost daily on their concerns about post-payment audits and recoupment. Post-payment audits involve an insurance company examining patient records for objective documentation that supports the care delivered. Without the proper documentation, the insurance company can demand re-payment.

You can avoid post-payment recoupment with the proper use of Outcomes Assessments in your documentation AND you can bill for this service utilizing 97211-25 typically at the second visit and every two-to-four weeks thereafter.

Warning: do not violate copyright laws! Copyright violations are serious crimes, with penalties up to $250,000 per incident.
